請假條常采用書信格式，比較正式，需寫明日期、稱呼、結尾和請假人的姓名，正文一定要寫明請假的原因。如：

Dear Miss Wang，
Im feeling very sad today and can not come to work．The doctor advised me to stay in bed for a few days．Now Im writing to ask for sick leave of three days．Ill come back to work as soon as I feel better．
